Maxlink Health TeleHealth has created a customizable Readmission Reduction Program (RRP) that will prove you guaranteed reduction of readmission over time. Our focus in RRP Program is to offer proactive care transition planning, patient centric post discharge care coordination and remote TeleHealth monitoring.
Maxlink Health uses a proven care system in which Maxlink Health RN Care Coordinator takes the hospital discharge orders, executes the orders and proactively coordinates care with patient's primary care provider and other community health providers. All patients are remotely monitored by our nursing call center.
