• Maxlink Health's TeleHealth Nurses will triage any patient critical care issues and consult with home health RN Case Manger or contact patient physician for only true clinical alerts.

• Maxlink Health's RN Care Coordinators monitor patients 7 days per week
• Outsource current teleheatlh program to Maxlink Health Telehealth
